How to apply for Duplicate GNM Marks Cards from KNMC?

If you have lost the GNM marks cards you can apply directly to KNMC together with required documents and fees and obtain Duplicate Marks Cards.  Complete guidance explained below:-.  

1.    How to write the Application?

(a)  Application should be on plain paper. It should contain your Full Name, Present Address, your active personal EMAIL ID and also your personal MOBILE NUMBER.

(b)  In the Subject, mention “Request for Duplicate Marks Cards”.

(c)  In the Reference, mention your Name, KNMC Registration Number, Date of Registration, and expiry date.

(d)  In case you have not received KNMC Registration Certificate, you have to obtain a letter in this regard, from the Principal of the Nursing School where you studied, and submit with the main Application.

(e)  Remember, if you are having KNMC Registration and it is already expired, you will have to renew it first, then only apply for Duplicate Marks Card.

(f)   Address the letter to Registrar KNMC, Trivandrum.  Give your introduction first.  Then inform them you lost your GNM Marks Cards (give details of  which ever marks cards, such as, 1st year, 2nd year, 3rd year, Internship etc.),  and it is lost forever and not traceable..

(g)  You have to pay KNMC Fees Rs 100 each per marks card thru NEFT/RTGS, and mention the UTR Number and Date in the Application.  If you are directly paying the amount to KNMC SBI Account, you have to enclose the Pay-in-slip as a proof of payment with this application.  Kerala Nurses and Midwives Council, maintain account with State Bank of India, Trivandrum City Branch, Account No. 57036987162, and IFSC No. SBIN0070028.

2.    Following documents to be submitted

(a)  Application  Letter duly completed and signed in
(b)  You have to make an Affidavit on Rs 200 stamp paper with attestation from First Class Magistrate, or from Notary.
(c)  Copy of lost Marks Cards, if available, please enclose.
(d)  Copy of SSLC Marks Cards (new), if old SSLC Book, then the page mentioned Date of Birth.
(e)  Calculate total duplicate marks cards needed and multiply x Rs 100, and pay by NEFT / RTGS.  Kindly mention UTR No. and Date in the application, or enclose the Pay-in-Slip (as proof of remittance) if you have deposited money directly into KNMC – SBI Account.

3.    Send the Application together with the above enclosures to following address by Registered Post/ Speed Post or visit KNMC and submit.
